Where each field comes from
Office listing (name, address, phone, website, rating, reviews): the office's public Google Maps listing.
Primary-care and other medical practices (for medical specialties): the federal NPI registry, by taxonomy and area, alongside their public Google listings.
Website check: a structured read of the practice's own website, with the date it was read.
Contact: the names found on the website, then business contact databases for a work email and direct phone. Emails are checked before they reach you.
